Our approach begins with referral and initial engagement, followed by a series of Mapping Sessions that build a detailed picture of each person's circumstances, needs and aspirations.
This information forms the basis of a personalised Health & Care Passport, which captures an individual's story, strengths, goals and agreed actions in one accessible document. Together, we develop a clear Action Plan that may include signposting to community resources, targeted Routes 2 Change interventions, wellbeing groups or peer support, ensuring that people receive the right support at the right time.
